For Providers

Welcome! We are providing free diabetes self-management classes to underserved communities in North Carolina. We do ask for an optional donation for the class series. We stress that if class participants cannot afford their medications or diabetes supplies, we do not wish for them to donate. Our classes are all taught by Certified Diabetes Educators. Please complete this one page patient referral and fax it to our office with a cover sheet. Please make sure to include all relevant information. No insurance or additional patient information is necessary.
