Diabetes Management Solutions primary funding partner to date has been the Blue Cross and Blue Shield of North Carolina (BCBSNC) Foundation. Being impressed with DMS's vision and excellent diabetes health outcomes, BCBSNC Foundation supported an initial pilot to bring mobile diabetes self-management education to rural providers. This was followed by a continuation grant to grow the initial diabetes health impact.
In 2006, Diabetes Management Solutions received a 5-year Reaching Communities Project Grant with the goal of taking the initiative statewide. Now in its fourth year, the Reaching Communities Project continues to expand.
Recognizing DMS's commitment to sustainability into the future, the BCBSNC Foundation most recently provided a three-year grant for implementing a capacity building program for self-sustainability.
Diabetes Management Solutions acknowledges with great thanks and gratitude the wonderful partnership we continue to enjoy with the Blue Cross and Blue Shield of North Carolina Foundation and their staff. Without them, this organization might not have grown into the organization it is today.
